Driving Miss Daisy
Provides a transportation service for seniors who need to get around the city for appointments, shopping, errands, social functions, day programs, etc. The service offers a door to door escort to ensure the senior’s safety. The senior must be able to self transfer into and out of the vehicle. The vehicle can accommodate folded up wheelchairs and walkers. A free consultation is offered for new clients.

Thunder Bay 55 Plus Centre and
55 Plus Programs at the West Arthur Community Centre
Offers a variety of support service programs for seniors such as the friendly visiting program, the telephone assurance program, and the walk-a-bit program. These programs, along with the onsite River Street Cafe and the drop in program, reduce the social isolation of the older adult by allowing them to engage in reoccurring social interactions. After an initial assessment, a volunteer is matched with the senior and the timing and duration of the programs can be negotiated. In addition, the center holds frequent health and wellness group education programs to discuss a variety of health related topics and issues relevant to the older adult population. There is no charge for many of these services and some programs are available for a nominal fee.

Alzheimers Society of Thunder Bay
The Alzheimer Society of Thunder Bay offers several programs and hosts a number of discussion and support groups for individuals with Alzheimer’s Disease and their families. Access to programs and support groups requires potential participants to contact the Alzheimer Society to complete and intake and registration course.
The First Link program allows individuals who have been newly diagnosed with Alzheimer’s Disease to be connected to programs and community services that would be of benefit to them. The First Link coordinator meets with the individual and their family to gain an understanding of the person with dementia and link the individual to health services and resources which would best benefit the client’s situation. The First Link coordinator will follow-up with the client throughout the Alzheimer’s Disease continuum to recommend services that would benefit the client during each stage. Additionally, the First Link program provides the client and his/her family with an understanding of the disease along with the new challenges it will present in the client’s life. The learning series is a progressive program that parallels the Alzheimer’s Disease progression. There are several topics which aim to increase the knowledge of Alzheimer’s Disease and offer strategies to aid with coping with Alzheimer’s Disease. The first learning session is geared for both the individual with dementia and his/her family to help with understanding the Alzheimer’s Disease diagnosis. Subsequent sessions are geared towards family members and friends caring for the individual with Alzheimer’s Disease. The sessions focus on communication strategies, understanding behaviour, and community resources available. A session also covers what options are available for care when the individual can no longer safely reside in the home. The Alzheimer’s Society is active in offering education to the public at health fairs or upon request from community groups or organizations. The Minds in Motion program is an eight week program that is offered once per week. The program is intended for people with early to mid-stage signs of Alzheimer's disease or other dementias and their care partners. The program combines physical activity and mental stimulation through a 2-hour session once per week. |

Hospice Northwest
Hospice Northwest provides compassionate support to individuals, their families, and caregivers living with a life limiting illness or those in need of grief and bereavement support after the death of a loved one. Trained volunteers provide support in the home, hospital, community, or long term care. Volunteers relieve caregivers, provide practical assistance, offer diversional activities, provide bedside support, assist individual and family to achieve meaning and satisfaction, and advocate for optimal care. Hospice Northwest offers the circle of friends program which allows for peer support and interaction in a group setting. One-to-one grief and bereavement support is also available. There is no cost to the user for these services. |

North of Superior Counseling Programs
The Senior Volunteer in Service (SVIS) friendly visiting service is available in all areas, pending volunteer availability, to support in providing companionship at home or take the senior into the community to attend appointments, complete errands or attend community events. Volunteers are trained, criminal-vulnerable sector checked, adhere to the principles of confidentiality and have regular support from the SVIS Coordinator. There is no cost to the user for these services.

Nishnawbe Gamik Friendship Centre
The Life Long Care Program is a service for Aboriginal people that offers a friendly visiting program, security checks by phone, and an elders program. Volunteers are available to provide companionship with older adults and take older adults into the community to medical appointments and complete errands. Popular social activities include outings for coffee, berry picking, and picnics. There is no cost to the user for these services.

District of Kenora Homes for the Aged
A variety of community of support services are offered which are primarily geared to older adults with the goal of providing independence through keeping people living in the community as long as safely possible. Programs that available include meals on wheels, congregate dining program in the community, an adult day program, LifeLine safety response, home help and maintenance, and a transportation service. A dedicated group of volunteers help deliver many of the programs. Some programs have a nominal cost involved.
“Club Day Away” is an adult day program which operates out of Pinecrest long term care in Kenora. This program attracts on average 8-10 older adults per day and operates from 9:30 to 3:00, Tuesday to Friday inclusive. Activities include outings in the community, exercises, social activities, music, and meal preparation. This program can be used for respite purposes. An application through the North West Local Health Integration Network(NWLHIN) is required along with a nominal cost to cover lunch and snack. Personal Support Workers run the program while volunteers assist with transportation (if needed) to and from the program. An adult day program of a similar nature is operated in Red Lake and Sioux Lookout, however, the adult day program only runs two days per week. |

Patricia Region Senior Services Inc.
A variety of services are available with the goal of keeping seniors living in their home as long as safely possible. Programs include meals on wheels, frozen meals, LifeLine personal response, supportive housing, social lunches, home help, yard maintenance, transportation, and a friendly visiting program. Volunteers are instrumental with running the different programs. The transportation program provides a subsidy for taxi services for individuals for transportation in the Dryden area only in order to travel to medical appointments, social events, and to complete errands. Nominal fees are charged for some services.

Wilson Memorial General Hospital
The Seniors Day Program is available for seniors and adults with disabilities. The program provides seniors with socialization, activities, and comradeship and is designed to accommodate all individuals, including those who are isolated in the the community. The program is an opportunity for socialization, as well an opportunity to provide families with respite. Activities include crafts, music therapy, exercise, entertainment, gardening, scenic drives, and monthly birthday celebrations.
Transportation is provided to the program (if required) by the Handi-transit service for a small fee. The program is run by a recreational coordinator and volunteers. There is nominal fee for participants. The program runs Tuesdays and Thursdays from 10 am to 3:00 pm. Coffee, tea and snacks are provided. |

Superior Elder Care
Superior Elder Care is a home support agency geared exclusively towards meeting the needs of seniors. A wide range of services are available including respite care, companionship visiting, and practical help. Home help can include laundry, meal preparation, housekeeping, personal hygiene care, and shopping. Services are available in personal homes, retirement residences, and long term care homes. A medical escort service is available 24 hours a day, 7 days a week to provide transportation to and from medical appointments. Personal support workers deliver many of the services and an hourly rate is charged for these services.

Nurse Next Door
Nurse Next Door is a private agency that provides homemakers, personal support workers, and nurses to render personal care and end of life care. A broad scope of both medical and non-medical care can be provided including live-in services. Services are available 24 hours a day, 7 days per week. A care consultation is mandatory before receiving services in order to build a care plan based on the older adult’s unique needs. An hourly rate is applicable to all the services and Nurse Next Door will work with veterans affairs.

Wesway
Wesway provides companionship services to older adults diagnosed with dementia. In order to be eligible for this service, the older adult must be living with a family member (ie. The older adult can not live independently). The older adult is allotted an amount of hours to use throughout the year to have a companion who can take him/her out in the community or provide in-home companionship. The companionship services offers respite for the family member(s) while facilitating inclusion of the older adult in community events and activities.
Companionship services are available on a mutually agreed basis between the companion and older adult, but can include weekdays, weekends, mornings, and evenings. Should Wesway not have a robust pool of staff in smaller communities (such as Marathon), Wesway allocates the older adult a sum of money in order to hire an independent contractor to provide the companionship services. The companion comes at no cost to the user, however, it is expected that the older adult (or family) cover the cost associated with any activities or meals for both the older adult and the companion. The waiting list for companionship services can range from immediate to one year. Wesway also operates a respite home in Thunder Bay where a maximum of 4 participants can stay for a weekend in order to provide respite to family members. Two staff members provide an array of programming at the respite home. |

Northwestern Independent Living Services Inc.
This community outreach program provides assisted living services for older adults in order to maintain maximum independence. A care plan is developed in collaboration with the program coordinator to determine what services best suit the needs of the older adult. Services provided include activities of daily living, personal care, help managing finances, assistance with utilizing community resources, attending appointments, shopping in the community, meal preparation, laundry, housekeeping, banking, exercises, dressing, transferring, and overnight respite Security checks may also be arranged in person or by telephone. Older adults have the choice to hire and manage their own attendant care staff or allow the program coordinator to provide and manage the attendant care staff. In order to be eligible, an older adult must be over the age of 60 and not on a waiting list for long term care. Although anyone can refer an older adult to this service, an admission selection committee has to approve each application. The program operates 24 hours per day, 7 days a week.

VON Thunder Bay District
VON provides an array of home support, physiotherapy, social work, and nursing services to clients. VON also provides respite care and palliative care services. Services can be rendered in the older adult’s home only. An initial consultation by a case manager will ensure that services are matched to meet the unique needs of the older adult.
VON operates an adult day program for older adults (average age= 85-92) diagnosed with mild dementia and have no risk of wandering. The program operates Tuesday to Friday inclusive and transportation is arranged through a dedicated bus operated by Hagi Transit or family can drop off and pick up the older adult. The adult day program accommodates 10 older adults and offers a rotating variety of programs such as cooking, crafts, music, physical activity, mind games, music, and current events. The adult day program removes isolation barriers by providing an outlet for socialization with other older adults. The program is run by two staff members and the program includes snacks and lunch. Referrals are necessary through North West Local Health Integration Network caseworkers and individuals currently living in long term care are ineligible. |

Rainy River District Home Support Program
The Rainy River District Home Support program offers a variety of services to allow older adults to continue living independently in their home. The home help program provides personal support workers to assist older adults with completing activities of daily living, personal hygiene, shopping, errands, housekeeping, laundry, and other chores. This program can also be used for respite purposes in order to temporarily relieve family members from their caregiver role or for friendly visiting purposes. This service operates from 7 a.m. to 8 p.m. with a minimum one hour appointment. There is a nominal fee for this service charged to the user.
The bath care program allows for older adults to safely receive a bath, should their home bathroom not be set up to execute a bath in a safe manner. This program adverts the risk for potential falls and other injuries associated with home bathing. The older adult is transported to the Rainycrest long term care home where he/she receives a bath in a whirlpool tub. There is a nominal fee (which includes transportation) for this service charged to the user. The adult day program allows those diagnosed with dementia, Alzheimers disease, or other cognitive disorders to participate in socialization and activities in a group setting. The goal of this program is for older adults to maintain their maximum level of functioning while providing respite for caregivers. The older adult is picked up from home in the LHIN van and returned at the end of the day. The program runs every Tuesday from 9 a.m. to 3 p.m. in the activation room at Rainycrest Long Term Care. Two personal support workers facilitate the program for a maximum of 10 clients. A CCAC referral is required to attend the program. A nominal fee is charged to cover a meal and snacks. |

Thunder Bay Police Service
The crimes against seniors unit assists older adults to recognize the risks and take actions to protect themselves against being a victim of elder abuse. Education is provided to older adults regarding marketing tactics and fraudulent scams that some companies routinely use to deceive the older adult for the purpose of exploiting their finances. Education is also provided to older adults regarding signs that they have been abused and the different forms of abuse such as physical, psychological/emotional, neglect, and financial. Although the crimes against seniors unit has a strong theme of prevention, older adults who feel they have been abused or are going to be abused in the future are encouraged to contact the crimes against seniors unit for assistance with determining the best course of action for an older adult’s unique situation.

St. Joseph’s Care Group
Manor House Adult Day Program
This program is exclusively for older adults with a diagnosis of dementia or Alzheimer’s disease. The purpose of the day program is to provide respite to caregivers while providing a comfortable social environment for older adults with dementia. The program runs during the week and is supported by a recreational therapist. A variety of programming is involved including baking, movies, music, exercise, recreational activities, and crafts. As the program provides respite for caregivers, the program’s goal includes allowing a person with dementia to be cared for at home rather than in an institution. A referral from the North West Local Health Integration Network (NWLHIN) is required to attend the program and a small fee is needed to cover the cost of food and activity materials. If required, transportation is provided by Hagi transit. Geriatric Assessment Program This program provides a comprehensive and holistic assessment of the older adult by both a nurse practitioner and a geriatrician. The assessment by the nurse practitioner takes place in the older adult’s home environment and includes a physical, cognitive, and functional assessment. The older adult is followed up by a geriatrician who reviews the assessment and makes recommendation in collaboration with the older adult’s primary healthcare provider. Once completed, a written report is made available to the client’s primary healthcare provider and follow-up is arranged with the geriatric assessment program as required. This program requires a physician referral and comes at no direct cost to the user. Psychogeriatric Resource Consultant Program This program provides a service to staff at long term care homes and mental health agencies in the form of training, education and consultation with the aim of improving the level of care delivered to older adults with dementia or Alzheimer’s disease. The psychogeriatric resource consultant is available to regularly meet with staff at the agency for educational topics or to consult on different issues, related to dementia, that the staff is challenged with at a given time. This service is available to the Thunder Bay and District area. Dementia Care Program The dementia care program provides an in-patient environment for older adults who display severe behavioural concerns related to dementia. A variety of health care disciplines for the interprofessional care team which aims to assess, treat, and stabilize the older adult’s concerns related to their dementia. With the permission of the healthcare team, families of the older adult can be involved in the treatment process. A physician or nurse practitioner referral is necessary for acceptance into this in-patient program. There is no direct cost to the user for this program. Seniors Behavioural Health Outreach Team This program explores the non-pharmaceutical dimensions of health for the older adult with behavioural issues related to dementia or other mental illness. Examples of topics explored during the assessment include behavioural issues, functional decline, safety, caregiver stress, education needs, and cognitive issues. A case manager is appointed to each older adult who will coordinate care by different health disciplines. Case managers are available to meet with older adults at their place of residence (including long term care homes) and will follow- up with the older adult as long as the services is required. A referral from a primary health care provider is required to access this program. There is no direct fee charged to the user for this service. Seniors Community Psychiatry Program This program provides psychiatric assessment, treatment, and follow-up which includes medication management to older adults with mental health challenges. Health care professionals are able to meet with the older adult at their place of residence (or long term care home) and provide necessary follow-up as required. A psychiatrist specializing in geriatrics will review current medication and provide advice on medication changes in collaboration with the older adult’s primary health care provider. A primary healthcare provider referral is necessary to access this service. There is no direct cost to the user for this program. Seniors’ Psychiatry Day Program The seniors’ psychiatry day program operates out of Lakehead Psychiatric Hospital and provides therapeutic programming services to older adults over 65 years of age who have a stable mental illness. The program includes both individual and group components and includes recreation, rehabilitation, leisure activities, and health promotion initiatives. A referral from a primary health care provider is necessary to attend the program and the intake coordinator approves successful applications. There is no direct cost to the user for this program. |

Centre for Education and Research on Aging and Health
The Centre for Education and Research on Aging and Health (CERAH) aims to facilitate high quality knowledge translation of research for the purpose of improving the health of an aging population. CERAH’s interests include the area of dementia and geriatric mental health. CERAH collaborates with many community partners to allow the product of research to take effect. CERAH also disseminates timely research and knowledge to healthcare providers by way of education sessions and publications. CERAH can be counted upon to support the development of high quality and evidenced based dementia care in Northwestern Ontario.

Canadian Mental Health Association- Fort Frances Branch
The Canadian Mental Health Association (CMHA) provides a variety of programs intended to assist older adults diagnosed with dementia to maintain independence and thrive in the community.
CMHA offers a case management program to older adults in order to assist with the navigation of the health and social services system. A support plan is created which reflects the unique needs of the individual and offers a diverse network of services to assist with maintaining independence in the community and continuing with a high quality of life. Supports and services can include help with navigating the health or mental health system, medication and symptom management, assistance with ADLs, health promotion, and education. A dedicated community support worker will establish a support plan and regularly follow the older adult to ensure continuity of care. The district mental health services for older adults program enables older adults to access specialized community-based geriatric services. In order to qualify, the older adult must be 60 years of age or older and be diagnosed with a mental illness, dementia, or Alzheimer’s disease. Examples of services offered include assessment and cognitive screening, counselling, care and treatment planning, referrals and advocacy, and cognitive bahavioural treatments. CMHA works closely with a geriatric psychiatrist from Toronto via videoconferencing in order to provide clients with assessments, case consultation, and education. This program allows older adults to be seen by a geriatric psychiatrist in their community without facing the geographical barriers of travelling. The older adults must be followed by a physician who is in support of consulting with the geriatric psychiatrist. A geriatric mental health worker facilitates the videoconferencing session and ensures that necessary follow-up and recommendations are provided. The psychogeriatric resource consultant program assists long term care homes and community support agencies with their older adult population having a diagnosis of dementia or Alzheimer’s disease. The psychogeriatric resource consultant provides the staff at the long term care homes or community support agencies with the knowledge needed to develop and critique care plans for clients who exhibit behavioural issues related to their mental health diagnosis. The psychogeriatric resource consultant can also provide education to staff tailored to the unique needs of the particular agency. |
